Adding an Alternate Address to Student

This form is to be completed by parents/guardians requesting that an alternate address be added to their student’s transportation file. An alternate address may be requested for morning transportation only, afternoon transportation only, or for both morning and afternoon transportation. This address will be used in place of the students home address. To support safe, efficient, and consistent routing, alternate addresses must be used on a regular five-day-per-week basis. Students are not permitted to maintain seats on separate buses for different days of the week.
